a request in writing including the following:
Name, address and authorized representative
of the company requesting service;
Desired Maximum Daily Receipt Quantity;
Term of service requested including
commencement and termination dates;
The proposed receipt and delivery points.
In the case of service through Saskatchewan, this is
McNeill (Alberta / Saskatchewan Border) and Monchy,
respectively.
